Jordan Health now provides health services for adolescents who formerly received primary care and family planning services through Threshold Center for Alternative Youth Services, which closed December 2013. Conveniently located along RTS bus route 2, we are now accepting new patients. The office is open Monday, Wednesday and Friday from 9 a.m. – 5 p.m.; Tuesday and Thursday 9 a.m. – 7 p.m. Call 585-454-7530 for more information.

The American Lung Association
The American Lung Association serves communities throughout New York state by providing numerous educational and support programs that help children and adults prevent and manage lung disease.
Not On Tobacco (N-O-T) is the American Lung Association’s (ALA’s) voluntary program for teens who want to quit smoking. It is the most researched, most widely used and most successful such program in the United States.

The Brown Square Refugee Health Program
Westside Health Services Refugee Health Program is a program at Brown Square Health Center focusing on providing assessments by experience family physicians of new refugees to the Rochester area, as well as continuing comprehensive refugee family health care. Refugees, asylees, and parolees of all ages who have been in the country 90 days or less are eligible for a health assessment at a contracted provider’s office. Services include health assessment screens for tuberculosis, hepatitis B, and stool ova and parasites (and lead screening in children). The health assessment includes a physical assessment and initiation of age-appropriate vaccines. The Refugee Health Program also provides dental, pharmacy, social work, financial counseling and assistance through “discount program,” HIV program, prenatal and immunization care.

CompassionNet
CompassionNet works through dedicated case managers and nurse practitioners to help families navigate the health care system and reduce suffering of children and teens with terminal illnesses by assisting families with the emotional, social, financial, and spiritual challenges associated with a child’s condition, and helping families cope effectively with a child’s illness and treatment. To be eligible, a child or teen must be diagnosed with a condition that places the child at significant risk for not reaching adulthood, OR have acute exacerbation of a chronic illness that places the child at immediate risk. Additionally, the child or teen must be 21 years of age or younger and be currently enrolled in a health insurance product managed by Excellus BlueCross BlueShield.

Epilepsy Foundation-Pralid Inc.
The Epilepsy Foundation is a non-profit human services agency that helps individuals and families of all cultures affected by epilepsy, traumatic brain injury, and related disabilities to manage and cope with their disorders by providing epilepsy education, information and referral, service coordination, vocational and residential services. Support programs are offered for family and friends of people with disabilities and education and information presentations are offered to community organizations, businesses, and schools. The Foundation also offers employment services, school-to-work programs, residential services, and a residential camp for children 8-15 years old with epilepsy and their siblings.

Starlight Pediatrics
Starlight Pediatrics (formerly called Foster Care Pediatrics) provides comprehensive primary pediatric health care to children from birth through 21 years residing in family foster care or supervised independent living in Monroe County and children on trial discharge from foster care. Services provided include preventive health care, management of chronic illness, acute care, adolescent health care, developmental screening and assessment, mental health screening, assessment and treatment, immunization administration, WIC re-certification, management of psychotropic medications, and consultation with child welfare around health issues for children in foster care.

Gilda’s Club Rochester
Gilda’s Club provides a warm and welcoming site for the cancer support community for adults, teens, and children with cancer and family members and friends can share their hopes and fears, their wisdom and information, and come together to learn how to live with cancer whatever the outcome. Gilda’s Club complements medical treatment and care by providing social and emotional. Support groups, dinners, arts and craft workshops, and play time for children are some of the many programs and activities that are available for members of Gilda’s Club Rochester.

Teens Living with Cancer
Melissa’s Living Legacy Teen Cancer Foundation is a national nonprofit organization dedicated to helping teens live with cancer through resources, support, and advocacy. The Rochester-area-based Teens Living Cancer program provides regular weekly activities for teens, both in the midst of treatment and beyond. The TLC Center hosts regular activities and programs for teens to help guide them through the challenges of cancer as well as a parent-networking groups. Additionally, the TLC provides hospital visits to serve as a newly diagnosed teen’s first introduction to the program and community outreach and education programs.
